Three MTG afflict cards side by side: Wildfire Eternal, a blue-armoured jackal-headed zombie wreathed in fire, Neheb, the Eternal, a horned zombie minotaur with a flaming spear, and Lost Monarch of Ifnir, a robed zombie raising a golden axe on a desert ridge, over a blurred background

MTG Afflict Explained: How Afflict Works, the Blocking Math, All 14 Afflict Cards Ranked and a $72 Neheb Deck (2026)

Afflict is a Magic: The Gathering triggered ability: whenever a creature with afflict becomes blocked, the defending player loses that much life. Afflict 3 means “Whenever this creature becomes blocked, defending player loses 3 life.” It came in with the Eternals of Hour of Devastation (2017), the zombified champions of Amonkhet. It turns blocking from a safe answer into a tax: the defender either takes the hit or pays life to stop it.

We pulled every afflict card from Scryfall, every deck count from EDHREC, and the printed power and toughness of all 17,692 Commander-legal creatures with numeric stats, then blocked each afflict creature with every one of them. The headline: when Neheb, the Eternal is blocked, the blocker dies and Neheb survives 84.9% of the time, and the defender has already lost 3 life. Only 5.4% of Commander creatures can block Neheb, survive and kill it.

There are 14 Commander-legal afflict cards: 10 creatures with the keyword, one Unfinity sticker sheet, and three cards that hand afflict to a whole team or a single creature. Ten of them come from one set, Hour of Devastation. Neheb, the Eternal takes 45.0% of all the Commander deck slots afflict cards get. This guide covers the rules word for word, the rulings that come up, the blocking math, every afflict card ranked by Commander play, what the keyword cost in stats, the team-wide granters, the forced-block cards that make afflict unavoidable, what does and does not boost it, legality in every format, and a $72.16 budget Neheb, the Eternal deck.

What is afflict in MTG?

The Comprehensive Rules (effective 25 September 2026) define it in rule 702.130:

702.130a Afflict is a triggered ability. “Afflict N” means “Whenever this creature becomes blocked, defending player loses N life.”

702.130b If a creature has multiple instances of afflict, each triggers separately.

That is the whole keyword. There is no cost, no condition on the blocker, and no choice for the attacking player. If the creature becomes blocked, the trigger goes on the stack and the defending player loses N life when it resolves. If the creature is not blocked, afflict does nothing and the creature deals its combat damage as usual.

The design point is the dilemma it creates. An attacking Neheb, the Eternal (4/6, afflict 3) offers the defender two options: take 4 combat damage, or block and lose 3 life anyway. Either way the defender loses at least 3. We call this the least the defender can lose: the smaller of the creature’s power and its afflict number. For most afflict creatures the two are close, so blocking barely helps.

How afflict works: timing, blockers and rulings

It triggers once, however many creatures block

Afflict uses the “becomes blocked” trigger, and rule 509.3c says that trigger “generally triggers only once each combat for that creature, even if it’s blocked by multiple creatures.” The Hour of Devastation release ruling says the same: if multiple creatures block a creature with afflict, afflict triggers only once. Triple-blocking an Eternal does not cost the defender three times the life.

The same rule also covers the odd cases. Afflict triggers if the creature becomes blocked by an effect, or by a creature put onto the battlefield blocking, as long as the attacker was unblocked at that moment.

It is life loss, not damage

The ruling is explicit: “Afflict causes the defending player to lose life; it’s not damage or combat damage.” This matters more than anything else about the keyword, because it decides which cards work with it:

  • Damage prevention (Fog effects, protection, “prevent all damage”) does not stop afflict.
  • Damage amplifiers (Torbran, Thane of Red Fell; Dictate of the Twin Gods; Fire Servant) do not increase it.
  • Cards that check “damage dealt this turn” (Spinerock Knoll, Knollspine Dragon) do not count it.
  • Cards that check “life lost this turn” do count it. Neheb, the Eternal is the obvious one.

It resolves before combat damage

Afflict triggers in the declare blockers step, so it resolves before any combat damage is dealt. The ruling: “If this loss of life brings a player to 0 life or less, that player loses the game immediately. A blocking creature with lifelink won’t deal combat damage in time to save that player.” A defender at 3 life cannot chump-block Neheb with a lifelinker to survive.

Multiple instances stack

Rule 702.130b: each instance of afflict triggers separately. Wildfire Eternal has afflict 4. Put it next to Lost Monarch of Ifnir (“Other Zombies you control have afflict 3”) and it has afflict 4 and afflict 3. Blocking it costs 7 life. Equip Dagger of the Worthy and it is 8.

Afflict in multiplayer Commander

Commander uses the attack multiple players option, so every opponent is a defending player. Rule 802.2a settles which one afflict means: the defending player for an attacking creature’s ability is “the player that creature is attacking, the controller of the planeswalker that creature is attacking, or the protector of the battle that player is attacking.” Afflict always drains the player you attacked, or the controller of the planeswalker you attacked. Attacking a planeswalker with Neheb still costs its controller 3 life when they block.

What afflict does not do

  • It does not trigger when the creature blocks. Only becoming blocked counts.
  • It does not make the creature harder to block, and it does not force a block. That is what lure effects are for (see below).
  • It does not do anything if the creature is removed from combat before blockers are declared.
  • It is not combat damage, so “whenever this creature deals combat damage to a player” triggers (Ammit Eternal, Lost Monarch of Ifnir) do not see it.

The blocking math: what it costs to block an afflict creature

We blocked every afflict creature with each of the 17,692 Commander-legal creatures that have numeric printed power and toughness, and sorted every block into four outcomes from the defender’s side. In every column the defender has also lost the afflict life.

Neheb: blocking costs 3 life and usually the blocker

Neheb’s 6 toughness is the important number. Only 5.4% of Commander creatures can block it, survive and kill it, and another 9.0% bounce off it with both creatures alive. 84.9% die without killing Neheb. So against Neheb the defender’s real choice is to take 4, or lose 3 and very likely a creature as well. Most players take the 4, and Neheb’s second ability counts it either way: every point of life your opponents lost this turn becomes {R} in your postcombat main phase.

Wildfire Eternal and Eternal of Harsh Truths: the dilemma cards

Two afflict creatures are built around the opposite trade. Wildfire Eternal is a 1/4 with afflict 4: if it is not blocked you may cast an instant or sorcery from your hand for free, and if it is blocked the defender loses 4. 54.6% of Commander creatures survive blocking it, but surviving does not save the 4 life. Eternal of Harsh Truths (1/3, afflict 2) draws a card when unblocked. On both, the defender is choosing which of two bad results to accept.

The small Eternals

Khenra Eternal and Merciless Eternal are 2/2s. Against them, 46.3% of blockers survive and kill the attacker, so blocking is usually right and costs 1 or 2 life. Spellweaver Eternal (2/1) dies to almost anything that blocks it. These are Limited cards; in Commander they only matter alongside Lost Monarch of Ifnir, which gives each of them an extra afflict 3.

Caveats

The table uses printed power and toughness only. It ignores combat tricks, counters and anthems, and treats first strike, double strike and deathtouch blockers like any others (796 creatures, 4.5% of the pool, have one of those printed). Ammit Eternal’s 5/5 is the most misleading line: it gets a -1/-1 counter every time an opponent casts a spell, so in a real game it is usually much smaller. The afflict loss is exact in every case, because nothing in the table changes it.

Afflict cards appear in 283,913 Commander deck slots on EDHREC. Neheb, the Eternal fills 45.0% of them, Cyberman Patrol 20.8% and Lost Monarch of Ifnir 18.9%. The three post-2017 cards together take 46.8%. Neheb is in 127,866 decks but leads only 4,016: almost every copy sits in the 99 of another red deck, played for its mana ability rather than for afflict.

The colour split is narrow. The nine Hour of Devastation afflict creatures are blue, black or red. There is no white or green afflict creature, and every one of the ten afflict creatures, Lost Monarch included, is a Zombie.

What afflict cost in stats

Was afflict paid for in power and toughness? We compared the nine afflict creatures with the other 87 creatures with numeric stats in Hour of Devastation, by mana value. Small samples, so read it as a direction, not a law.

Mana valueAfflict creatures (n)Avg P/TOther Hour of Devastation creatures (n)Avg P/TPower difference

5+24.50 / 5.00214.76 / 4.76-0.26
All92.78 / 3.33872.75 / 3.00+0.03

At mana values 2 and 3 the afflict creatures are bigger than their set-mates (+0.42 and +0.53 power, and +0.92 toughness at 3). Overall the averages are almost identical: 2.78/3.33 against 2.75/3.00. The one row where afflict creatures are smaller is mana value 4 (2.00 against 3.00 power), and both cards there pay for a second ability: Frontline Devastator’s firebreathing and Wildfire Eternal’s free spell. Afflict itself was costed as close to free.

Cards that give afflict to a team

Three cards printed after 2017 grant afflict, and they are why the keyword still shows up in Commander. Each one is a tribal card; the table shows how many Commander-legal creatures share the type that benefits.

Lost Monarch of Ifnir and the Eternals

Lost Monarch of Ifnir (Aetherdrift Commander, 2025) is a 4/4 with afflict 3 that gives every other Zombie you control afflict 3. All nine Hour of Devastation afflict creatures are Zombies, so each one gets a second instance on top of its own. There are 608 Commander-legal Zombies, 402 of them mono-black or colourless. That is the deepest afflict deck Magic can build, and the Monarch’s second-main-phase trigger returns a creature card from your graveyard whenever a Zombie connected.

Cyberman Patrol

Cyberman Patrol (Doctor Who Commander, 2023) is a 2-mana colourless 2/2 that gives every artifact creature you control afflict 3, itself included. With 1,245 Commander-legal artifact creatures it fits any artifact deck, which is why it sits in 59,167 decks. It is also the priciest afflict card at $14.24.

Lazotep Sliver

Lazotep Sliver (Commander Masters, 2023) gives Slivers afflict 2 and makes a Sliver Army when a nontoken Sliver dies. In Sliver decks, which usually stack evasion and pump, it punishes the blocks that do happen.

Dagger of the Worthy is the only other granter: +2/+0 and afflict 1 for 2, equip 2. It is the cheap way to stack one more instance on Neheb.

Forced blocks: the cards that make afflict unavoidable

Afflict is only a choice for the defender while they are allowed not to block. “Must be blocked if able” and “all creatures able to block it do so” (lure effects) take that choice away: as long as the defender has an untapped creature that can block, the afflict life is gone. We counted 72 Commander-legal lure cards. 42 are mono-green, 9 colourless and 7 mono-red; there are no mono-blue, mono-black or mono-white ones. The most-played 12:

For a red afflict deck the useful ones are the colourless Equipment (The Masamune, Nemesis Mask) and red’s own “must be blocked if able” creatures (Fear of Being Hunted, Raphael, Ninja Destroyer, Goblin Fire Fiend, Jurin, Leading the Charge). The Masamune is the best fit for Neheb: while attacking, the equipped creature has first strike and must be blocked if able. Neheb then deals 4 first-strike damage to its blocker, which kills anything with toughness 4 or less before it can hit back, and the defender has already lost 3. Note that a lure effect still triggers afflict only once (rule 509.3c), even when Nemesis Mask forces every creature to block.

What boosts afflict, and what does not

Because afflict is life loss, half the usual red damage package does nothing for it. From the deck below:

Neheb, the Eternal: the only afflict commander

Neheb, the Eternal is the only legendary afflict creature. Neheb is a 5-mana 4/6 with afflict 3 and “At the beginning of each of your postcombat main phases, add {R} for each 1 life your opponents have lost this turn.” Every point counts: combat damage, burn and afflict. A turn where Neheb is blocked (3) and a Guttersnipe trigger hits three opponents (6) adds 9 red mana after combat. EDHREC lists 4,016 decks with Neheb as commander. Their most common cards are burn and “deals damage to each opponent” engines: Guttersnipe, Flame Rift, Fiery Confluence, Comet Storm, Thermo-Alchemist, Jaya’s Immolating Inferno. The afflict creatures are rare in them; Wildfire Eternal is the only one in more than a quarter of budget Neheb decks.

That is the gap the deck below fills. It keeps the proven burn engine and adds the afflict creatures, Dagger of the Worthy and the forced-block cards, so Neheb’s own attack costs an opponent 3 or 4 life every turn and fuels a big postcombat X spell.

How the deck plays

  • Turns 1 to 4: ramp and pingers. Sol Ring, Arcane Signet, Mind Stone, Fire Diamond, Ruby Medallion and Hazoret’s Monument get Neheb out on turn 4 or earlier. Kessig Flamebreather, Coruscation Mage, Electrostatic Field, Unruly Catapult and Guttersnipe turn every spell into damage to each opponent.
  • The attack. Neheb attacks every turn. Swiftfoot Boots or Lightning Greaves protect it. The Masamune or Dagger of the Worthy make blocking it worse. Manticore Eternal, Frontline Devastator and Wildfire Eternal add more afflict pressure, and Fear of Being Hunted and Raphael, Ninja Destroyer have to be blocked if able, which soaks up blockers.
  • Postcombat. Neheb adds {R} for every life point lost. Spend it on Fireball, Banefire, Comet Storm, Fall of the Titans or Jaya’s Immolating Inferno, or on Magus of the Wheel and Big Score to refill.
  • The finishers. Heartless Hidetsugu halves every life total (yours too). Fiery Inscription and Dictate of the Twin Gods turn the spell chain into a kill, and Apex of Power is ten mana from your hand.

Afflict compared with other block-punishing keywords

Afflict is one of several keywords that make blocking a creature cost something. Flanking shrinks every blocker by -1/-1 before damage. Bushido pumps the creature when it blocks or becomes blocked. Menace makes the defender commit two blockers. Afflict is the only one that hits the player instead of the blocker, which is why it ignores first strike, deathtouch and big butts. For the combat-damage keywords those blocks run into, see our first strike, deathtouch and trample guide. If you want the Lost Monarch route instead, the best Zombie commanders guide covers the black Zombie decks it slots into.

Afflict FAQ

What does afflict do in MTG?

Whenever a creature with afflict N becomes blocked, the defending player loses N life. It triggers in the declare blockers step, before combat damage.

Does afflict trigger once per blocker?

No. It triggers once per combat no matter how many creatures block (rule 509.3c and the Hour of Devastation release ruling).

Is afflict damage?

No. It is life loss. Damage prevention does not stop it, and damage doublers like Torbran, Thane of Red Fell and Dictate of the Twin Gods do not increase it.

Can a lifelink blocker save the defending player?

No. Afflict resolves before combat damage, so if it takes a player to 0 life they lose before the lifelink damage happens.

Do multiple instances of afflict stack?

Yes, each triggers separately (rule 702.130b). Wildfire Eternal with Lost Monarch of Ifnir in play has afflict 4 and afflict 3, so blocking it costs 7 life.

Which player loses life in multiplayer?

The player the creature is attacking, or the controller of the planeswalker it is attacking (rule 802.2a).

What is the best afflict commander?

Neheb, the Eternal is the only one. For an afflict-heavy deck without Neheb, use a black Zombie commander with Lost Monarch of Ifnir in the 99.

Is afflict an evergreen keyword?

No. It was printed on ten cards in Hour of Devastation (2017). Since then only four more cards have used it, none of them in a Standard set: Lazotep Sliver and Cyberman Patrol (2023), Lost Monarch of Ifnir (2025) and one Unfinity sticker sheet.

What colours is afflict in?

Blue, black and red, plus colourless granters (Dagger of the Worthy, Cyberman Patrol). There are no white or green afflict cards.

Is afflict good in Commander?

On its own it is modest: 3 life against 40 is small. It gets strong when the life loss is doing a second job: Neheb’s mana, a forced-block Equipment, or Lost Monarch stacking a second instance on every Zombie.
